Job summary We are currently recruiting for a further Lead Reablement worker based in Signet Court, Cambridge. The Reablement Service provides care at home to people aged 18 upwards. Support is delivered in an enabling way so that people can maximise their independence. Reablement support is a short term service. The Lead Reablement workers play a crucial role in supporting individuals transitioning towards independent living. They are the first point of contact for service users, working closely with Occupational Therapy teams and other health professionals to set personalized goals. These goals guide the service user through their journey until they achieve independence. Additionally, Lead Reablement workers act as key contacts, referring users to community support services and continuously monitoring their care throughout the reablement process.
