20-21 p/h (Umbrella) Enfield is currently searching for an experienced Rapid Assessment and Resettlement Practitioner to join their Out of Hours team. Main Duties: Conduct comprehensive assessments and develop Resettlement Plans and Personal Housing Plans for individuals who have experienced street homelessness. Collaborate with Specialist and Housing Coordinators to ensure effective implementation and delivery of housing solutions and support services. Maintain accurate and up-to-date service user documentation and enter relevant data into IT systems. Provide support and practical assistance to service users, working as part of a team to meet their needs and achieve positive outcomes. The Candidate Will Have: Experience working with people who have experienced homelessness, with a focus on resettlement planning and housing support. Knowledge of relevant housing and homelessness legislation, policies, and procedures. Ability to work flexibly, including shift work (nights and weekends), and collaborate within a multi-disciplinary team to meet service goals.
