This is an exciting opportunity to join a dynamic and motivated Pain Management Service at Chelsea and Westminster Hospital. This is an established service that provides highly specialised services to paediatric and adult patients with acute and chronic pain, within a multidisciplinary setting.

We are looking for a registered paediatric nurse who has a genuine interest in Pain Management. As a Paediatric Associate Clinical Nurse Specialist in Pain Management, you will be expected to support the development and delivery of the clinical paediatric pain service for both inpatients and outpatients.

Previous pain management experience is not essential, but the individual must demonstrate excellent communication skills and be willing to develop their skills. Additionally, sufficient knowledge will be required to manage complex pain situations. The individual must be motivated to develop and support progression of the service. There is great scope in this role to grow both personally and professionally.

Main duties of the job

The Paediatric Associate Clinical Nurse Specialist in Pain Management will be expected to support the development and delivery of the clinical paediatric pain service, as well as the rest of the pain management service.

The applicant should be a registered paediatric nurse educated to first degree level in a health-related subject. The individual should be undertaking or willing to undertake appropriate post-registration education and development within the area of specialist practice.

The role of the Paediatric Associate Clinical Nurse Specialist in Pain Management includes the following supervised activities:

Managing a caseload of specialist patients both in the inpatient and outpatient environment. Receiving referrals. Providing assessment, planning, delivery and evaluation of specialist care within defined guidelines and protocols (both Local and National). Referring to other disciplines and agencies. Involvement in training and education for the multidisciplinary teams. They must also be willing to develop their own knowledge and skills. Participating in audit and research activity. Disseminating up-to-date evidence-based practice. Ensuring effective communication between all service providers. Assisting the Pain Consultant with adolescent outpatient clinics including inputting outcome data into a database.

Key Responsibilities

To contribute to the smooth running of the inpatient Pain Management Service.

Provide holistic assessments, plan and conduct pain treatment. Follow-up planned treatment, under the supervision of expert colleagues and within agreed guidelines and protocols.

This involves undertaking daily specialist ward rounds and twice weekly multidisciplinary ward rounds.

To carry the inpatient pain bleep once competent and respond to urgent or non-urgent situations. If required, offer specialist advice in accordance with agreed guidelines and protocols. This may involve: dealing with patients in severe pain or with suboptimal pain management, managing side effects and administering or recommending the appropriate treatment. Finally, recognising the unwell patient who requires referral or urgent medical attention and escalating as appropriate.

To draw on both their own and others' knowledge in decision-making to determine evidence-based interventions within guidelines and actively monitoring these.

To act as an advocate for the patient in treatment-related decisions and choices.

To refer a patient who requires the administration of an epidural bolus or IV opioid for severe pain to an Anaesthetist and following a period of supervision from the CNS & Consultants, deliver this treatment according to agreed guidelines and protocols. To seek medical advice in patients with respiratory depression/over dosage of opioids.

To co-ordinate with an Anaesthetist, to provide a 24-hour inpatient pain service for patients in all clinical areas (adults and paediatric). Provide handover information to the on-call Anaesthetist for out of hours care where necessary.

To have supervised input into the delivery of the outpatient Pain Management Service and assist the Consultants and other CNSs in the development and future delivery of paediatric outpatient Pain Management Services.

Participate in Pain Management Service multidisciplinary team meetings.

To use effective communication and interpersonal skills to provide information, explanation and reassurance to both patients and families. Furthermore, build an environment of trust, promoting patient empowerment.

To maintain accurate and contemporaneous records of patients' assessment, care, treatment and follow-up.

To participate in providing specialised aspects of patient care and treatment, working in partnership with others.

To provide holistic assessment, care and treatment to patients under the supervision of expert colleagues and within agreed guidelines and protocols and plan/conduct appropriate patient follow-up.

To address patients' health promotion and prevention needs through comprehensive assessment of individuals for risk factors and early signs of illness. Key Working Relationships Consultant Anaesthetists - Pain Management Service
