We are seeking a highly committed and motivated individual who will ensure that we are providing a patient-focused discharge service to patients in our Acute areas - Acute Medical Unit and Emergency Services. We are looking for a dynamic and motivated individual with previous experience of managing a Discharge service, excellent communication skills and an innovative and flexible approach. Main duties of the job
This post is specifically responsible for the leadership within the IDS department and the development of processes to reduce the length of stays and to facilitate the timely and the safe discharges of the patients with complex care needs. The post holder will also lead a team of nurses in continuing to build a discharge service that works across the Trust to ensure safe, timely and effective transfer of patient care on discharge. They will have the ability to build relationships with staff across all disciplines between both the hospital and within the community services. They will also provide a strategic overview of the service and work to develop effective systems to reduce the number of delayed discharges. They will also deputise for the IDS Senior Nurse when requested. Job responsibilities

Knowledge and Qualifications
Registered Nurse Part 1, 2 or relevant section of the register Honours degree (Level 3/ FHEQ Level 6) completed or 50% achieved; commitment to completion Teaching course Relevant clinical course Recognised Leadership course NVQ assessor Experience
Minimum 2 years' relevant clinical and managerial experience in a senior position Proven track record of leadership Evidence of clinical competence and regular clinical practice Skills and Abilities
