The purpose of this role is to be part of a small team of senior nurses who are responsible for the clinical coordination of care for patients and families referred to Rennie Grove Peace. Main Duties of the Job
Being the point of contact for patients, families, clients, and professionals who contact the hospice, dealing with all clinical enquiries. Providing clinical oversight and triage for referrals to ensure that patients and family members get the right Rennie Grove Peace service in a timely manner. Developing the Coordination Centre's nursing role to ensure it provides a responsive, evidence-based, and high-quality service to beneficiaries and professionals. Working as a Specialist Palliative Care Clinical Nurse Specialist (CNS) in a different Rennie Grove Peace clinical setting on a monthly basis to maintain and develop skills. Promoting the Rennie Grove Peace service to external Health and Social Care professionals and members of the public. Undertaking activities such as audits and case review to improve the Coordination Centre service. Contributing to the wider strategic development of the hospice and translating local and national strategy relating to hospice care. Acting as a Rennie Grove Peace Hospice Care ambassador, demonstrating our values and promoting the specialist care we provide. About Us

Demonstrate clinical excellence and maintain consistently high standards of nursing and palliative care. Act as a role model demonstrating high standards of care and providing clinical leadership for colleagues. Advise and support patients with complex needs and their families at home, incorporating physical, psychological, social, and spiritual aspects of care. Effectively allocate and support team members to implement the care plan, monitoring and evaluating the care provided. Ensure all written and verbal communication is constructive, effective, accurate, and appropriate. Develop and maintain effective working relationships with community professionals and support services. Demonstrate a research-based approach to nursing and participate in ongoing Rennie Grove Peace research. Identify risk and lead risk management involved in patient care and lone working. Person Specification
Experience
Registered Nurse. Degree Level or modules in Palliative Care or another relevant subject. Evidence of continuous personal and academic development. Proven experience of managing people/strong clinical leadership. Proven experience of effective team working. IT literate including a good knowledge of Excel, Word, and a clinical IT system. Excellent written and verbal communication skills. Good organisational skills. Teaching and presentation skills. Advanced assessment skills and specialist symptom control knowledge to plan, implement, and evaluate nursing care. Commitment to Rennie Grove Peace hospice care mission and aims.
