An opportunity has arisen within the Gastroenterology Care Group on the AUH site for a Specialist Nurse in Gastroenterology, working as part of the complex polyp team. This post will support the new regional complex polyp service for Cheshire & Merseyside, covering a population of approximately 2.7 million. There are 5 gastroenterology consultants with specialist interest in complex polyps and the team is leading the way in management of these lesions. The post holder will be an integral member of the established complex polyp team working alongside the existing Lead nurse and the team to enhance the service currently offered. This post would be suitable for a Registered nurse with substantial post registration experience and with demonstrable experience of working within gastroenterology or colorectal surgery. This job is to support the Complex polyp, EMR and Endoscopy service at the Aintree University Hospital Trust. It is expected that the specialist nurse will be a pivotal part of the GI endoscopy service for complex polyps/EMR & ESD. Complex Polyp service will require working effectively as part of the multidisciplinary team to help deliver a high quality complex polyp and EMR/ESD service. This will involve database management, welcoming tertiary patients into our service, pre & post assessment, MDT preparation and presentation, liaison between multidisciplinary groups, follow-up and hand-over of patients to relevant services. This requires a wide knowledge base and advanced clinical decision making skills. They will be integral to the Regional complex polyp MDT in supporting patients and enhancing pathways into and out of the service. It will work closely with Bowel Cancer Screening Services throughout the region. The post holder will be a role model for professional practice, exercising accountability and providing support and leadership within the endoscopy team. The post holder will contribute to service development, improvement and management, demonstrating a proactive approach to issues and service quality. Job responsibilities

Perform specialist assessment of patients nursing needs, plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills. Lead clinical care by managing a patient caseload providing expert assessment, planning and evaluation. Co-ordinate and manage nurse-led clinics (within Speciality). Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements. Establish and maintain effective communication with patients and carers/relatives. Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery. Recognise changes in patients condition that require the intervention of others and refer on as appropriate. To support, assist and encourage the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of case management. To lead the specialist nursing contribution to the service, in the delivery of high standards of patient care.
