Job summary We have an exciting opportunity for an Advanced Nurse/Clinical Practitioner to join our super GP practice in Crawley, West Sussex. We aim to ensure that the highest levels of care are delivered to all our patients, particularly in areas such as minor illnesses and acute, on the day appointments. This is a highly supportive practice with a long-standing team including GPs, Paramedic Practitioners, Advanced Nurse Practitioner, Practice Nurses, HCAs and many more Additional Roles. Main duties of the job To assess, diagnose, treat, refer or signpost patients who attend surgery with undifferentiated or undiagnosed conditions relating to minor illness. To undertake home visits, primarily for acute conditions, as assessed by your duty doctor, and within the boundaries of professional qualifications. The post holder will use advanced clinical skills to provide education to patients, promoting self-care and empowering them to make informed choices about their treatment. About us Bridge Medical Centre comprises 4 GP Partners, a nursing team of 6, Paramedic Practitioners, an Advanced Nurse Practitioner and a vast supportive administrative team. The Practice is part of the Crawley Care Collaborative PCN who are each members of the Alliance for Better Care Federation. Our clinicians work independently as well as part of a team collaborating with our own clinical and administrative team whilst also working closely with the Additional Roles from the PCN. These roles comprise Clinical Pharmacists, Dieticians, First Contact Physiotherapists, Care Co-ordinators and Social Prescribers. We are a sociable Practice and like to socialise and celebrate our success. Job responsibilities Diagnosing and treating patients presenting with minor illness Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team. Patients with problems needing referral to secondary care should be discussed with the registered GP before making such referral. Ensure clinical Practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations. Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary. To undertake home visits, primarily for acute conditions, as assessed by your duty doctor, and within the boundaries of professional qualifications. To communicate at all levels within the team ensuring an effective service is delivered. To maintain accurate, up-to-date healthcare records appropriate to the consultation. Ensure evidenced-based care is delivered to the highest standards. Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service. To participate in the audit process, evaluation and implementing plans and Practice change in order to meet patient need. Contribute positively to the effectiveness and efficiency of the team and work colleagues. Pathological specimens and investigatory procedures Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs. Administration and professional responsibilities Participate in the administrative and professional responsibilities of the Practice team. Ensure System 1 is kept up to date, with accurate details recorded and amended. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice. Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit. Attend and participate in Practice meetings as required. Training and personal development Training requirements will be monitored by yearly appraisal and will be in accordance with Practice requirements. Personal development will be encouraged and supported by the Practice. It is the individual's responsibility to remain up to date with recent developments. Participate in the education and training of students of all disciplines and the induction of all members of the Practice staff where appropriate. Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development. If it is necessary to expand the role to include additional responsibilities, full training will be given. The post-holder will participate in any training programme implemented by the Practice as part of this employment. Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work. Person Specification Skills/Abilities Essential An excellent standard of written and verbal English. Excellent listening skills. Ability to make autonomous decisions. Ability to assess, minimise and manage risks and defuse stressful situations, ability to stay calm. Competent use of IT systems, Excel, Word and Outlook. Desirable Use of SystmOne. An understanding of Chronic Disease Registers and QOF. Experience Essential Experience of working in an urgent care/emergency setting. Experience of working in primary care setting. Ability to work within a team to provide the best outcome for the patient. An ability to use own judgement, resourcefulness, common sense and local knowledge, to respond to enquiries and requests while adhering to Practice limitations. Recognise and work within own competence and professional code of conduct. Qualifications Essential Qualified Advanced Nurse/Clinical Practitioner. Evidence of appropriate knowledge base. Desirable Independent Prescribing qualification. Other Essential Flexible attitude to work. To be willing to work later/earlier as duties/events require. Clean driving licence.
