Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team. The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager. The responsibilities will include: Adult Assessment and Care Management Services Provide assessments under the Care Act 2014 and work with young people transitioning from Children’s services to Adult services, and beyond Provide early intervention to support adults and carers to prevent social admissions to hospital Visit adults where necessary and complete assessments and support plans to meet needs Alert the Senior where there are safeguarding concerns Work according to the relevant legislation, supporting Older People and younger adults over the age of 18 with physical disabilities or sensory impairment, as well as their carers Assess and review adults in receipt of care and support and manage and facilitate safeguarding enquiries Maintain excellent report writing and computer skills; record keeping is vital Be flexible, committed, and hard working with a willingness to work to quality standards Work within a multidisciplinary framework and develop close working relationships with partner agencies 37 hours per week (flexible by negotiation) 3 Month Temporary Contract Start date subject to DBS and reference clearance. The successful Qualified Social Worker candidate will need to hold a Degree or equivalent, must be registered with Social Work England, have a current DBS, and be a car driver.
