Main Duties of the Job
Help support and manage people with diabetes referred to the service within a defined population as well as provide diabetes advice and guidance to other health care professionals as required.
Use clinical skills for assessing and treating patients with diabetes.
Provide a point of contact for the patient population offering specialist advice and referral routes.
Communicate with service users referred into the service with respect and empathy, using a person-centred approach and motivational interviewing skills to promote self-care and self-efficacy.
Monitor and review people with Type 1 or Type 2 diabetes in the community via virtual, telephone, face-to-face clinics or group education regarding symptom control, prescription, and titration of medication adhering to local guidelines.
Deliver diabetes care in a clinic setting: face to face and remotely via video, telephone, and email following clinical guidelines and triage guidance.
