We have an exciting opportunity for an experienced Senior Support Worker to join our social care team in Mental Health Services for Older People to assist us with the smooth running of the service. As a healthcare support worker, you'll be at the very heart of everything we do - it's a vital role that keeps the service going. You will work under the supervision of a healthcare professional, supporting them and helping patients on their journey back to full health. Main Duties of the Job
You will be expected to: Assess need, plan, implement/commission, and evaluate care for a number of people that are allocated to you; this will include decisions regarding risk and risk management. Regularly work alone, without direct supervision, while being accountable to a qualified member of the team. Exercise communication skills, both verbal and written, and engage with acutely distressed people in challenging and unknown situations. As a staff member, you will represent Hertfordshire Partnership NHS Foundation Trust (HPFT) and play a vital role in fostering positive relationships and communication between the Mental Health Trust and clients, their carers and relatives, and other agencies, charities, and providers. Complete an assessment of care needs (primarily social care) and risk assessment of patients presenting with a variety of clinical conditions. Develop, implement, and evaluate programmes of care without direct supervision (though clinical supervision is implemented as per policy to support you). Provide advice, guidance, and education to clients and their carers regarding social care provision and policy. Participate fully in providing quality care in line with local and national guidelines. Act as a role model in providing a service for people with mental health problems. Participate in team audit projects associated with the service. Challenge practices that compromise high-quality care for older people. Advise Team Manager of any resource shortfalls. Partake in continuing professional development that benefits the service. Document all activity on Care Notes (PARIS), ACSIS, and Outlook, and utilize Employee Online, Online Expenses, and other computerized systems. Person Specification

The ability to organize and prioritize own workload within specified guidelines. Proficient in diary management to coordinate appointments. Recognize people's rights to privacy and dignity, treating every person with respect. Good range of problem-solving and information-gathering skills. Experience of adapting to change and managing work in a changing environment. Evidence of flexibility and initiative, sensitivity, and enthusiasm. Ability to make clinical decisions or seek guidance from a senior colleague. Communication Skills
