To provide Speech and Language Therapy Services to stroke patients with speech, language, communication needs and their families within Harrogate and Rural District NHS Foundation Trust.

Post holders will be based at Knaresborough Road Health Centre, a short walk from Harrogate and District Hospital NHS Foundation Trust.

Main duties of the job

The post holder independently provides a comprehensive clinical service for stroke patients with Speech, Language and Communication needs within the Harrogate and District NHS Foundation Trust footprint.

Caseload management is carried out independently.

Close liaison is maintained with health and social care professionals to provide integrated and relevant care packages.

To provide training, direct and indirect support to families and health professionals.

The post holder is a member of the Speech and Language Therapy Team within Harrogate and Rural locality of HDFT. This includes providing supervision to Speech and Language Therapy Assistants as necessary.

To have extensive knowledge of all types of speech, language and communication disorders across a wide age range of stroke.

To assess clients and work directly and indirectly to deliver Speech and Language Therapy treatment programmes.

To carry out assessment as appropriate and make differential diagnosis.

To provide appropriate intervention / treatment and evaluate outcomes.

To provide advice to others regarding the management and care of patients / clients with communication difficulties (e.g. with carers).

To demonstrate clinical effectiveness by use of evidence-based practice and outcome measures.

To produce reports / targets regarding patients' needs and summarising proposed care plans.

Speech and Language Therapy graduate with membership of HCPC Relevant experience at undergraduate level of working with adults including stroke Experience of working in the community Car driver

Desirable

Registered Member of Royal College of Speech and Language Therapists Knowledge and use of Systm1 Knowledge and experience of stroke pathway Delivering speech and language therapy interventions Working knowledge of the National Stroke Strategy
