The Role

Partnering our discharge coordinators, you will ensure that admissions and discharges are seamless. Ensure all clinical assessments are completed within 24 hours of referrals, to minimise bed blocking within local hospitals. Ensure referred patients are suitable for our rehabilitation programme and colleagues are fully briefed on their needs. Oversee the implementation of care plans, making certain we deliver the highest standards of clinical support and outstanding patient-centred services. Supporting residents with a range of complex needs and dependency levels.

About You

An NMC Registered Nurse. Understands clinical assessment tools like Waterlow and MUST. Experienced in writing comprehensive care plans.
