Ranworth PCN are seeking an experienced practice nurse to work at their Green Elms Surgery on a full-time, permanent basis to support the provision of high-quality healthcare and diverse patient list. Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population, responsible for a number of clinical areas including infection control, chronic disease management, well women and well man clinics and health promotion. Main duties of the job

Support the production of the IPC annual statement in conjunction with the nurse manager. Develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice. Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice. Develop, implement and embed health promotion and wellbeing programmes. Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health. Diagnose and manage acute and chronic conditions. Review patient medications to enhance compliance. Assist GPs with minor surgery. Support the clinical team. About us

Qualifications

Registered nurse - current NMC registration with an active PIN. Mentor or teaching qualification. Post graduate diploma or degree (Chronic disease management). Family planning qualification. Experience

Experience working in a primary care environment. Chronic disease management experience. Infection and preventative measures experience. Experience of continuing professional development (CPD). Flexible to working outside office hours. Experience of working as a community nurse. Disclosure and Barring Service Check
