This post is open to individuals with occupational therapy backgrounds who have an interest in homeless health issues. They will provide clinical expertise and case management for patients referred to the service, with an understanding of the complex tri-morbid factors that the patient cohort experiences.
Main Duties of the Job The post holder will engage in significant partnership working aimed at improving hospital discharge for individuals and groups of patients. This partnership will include NHS services, GPs, housing departments, social care, and voluntary sector organizations. Responsibilities include ensuring processes for holistic assessment, planning, and provision of interventions.
The post holder will work alongside a Specialist General Practitioner, a Homelessness Nurse Specialist, two Housing Officers, an Adult Violence Practitioner, and a Care Navigator to provide additional support to homeless people. They will support the Homeless Team in the enhanced management of homeless patients on the wards and provide direct support to medical and surgical teams, therapists, and wards in discharging patients safely and timely.
The team works closely with staff across all disciplines between hospital and community to ensure a safe, timely, and effective transfer of patient care on discharge. The aim is to improve the health of homeless people, provide holistic coordinated care, reduce delayed discharges, and minimize premature discharges of homeless patients to reduce future unscheduled admissions and ED attendances.

Person Specification Qualifications
Diploma/Degree in Occupational Therapy
Registered with Health Professionals Council as Occupational Therapist
Post Graduate Training
Teaching and Assessing course or mentorship course
To have or be currently working towards a Master's degree
Specialist qualification in a relevant area, e.g., addictions
Experience
Experience working with complex clients
Experience of undertaking holistic health assessments and assessing mental health and social care needs
Experience of working in MDT
Experience of liaison with statutory and non-statutory services
Experience working with marginalized groups
Skills
Warm, friendly manner
Excellent oral and written communication skills
Confident in problem solving and making independent decisions

Demonstrated ability to work successfully with patients with personality disorder and complex trauma
