The successful candidate will be a part of the Memory Assessment Service where they will work with those who are experiencing cognitive changes and have concerns regarding Dementia. The successful candidate will undertake responsibility of the Memory Assessment Service in the absence of the manager. They will be required to divide their time equally between a clinic and community care setting due to the large rural geographical area with limited transport links and problematic mobile phone reception. It is necessary to work autonomously and conduct patient home visits. The successful candidate is expected to participate as a full member of the multi-disciplinary team, working with people who present with possible organic mental health problems. Taking responsibility for a caseload of patients and initial comprehensive assessments, delivering diagnosis and formulating treatment plans and initial review. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply. About us
