GBP38,000 - GBP46,000 per annum with an extensive benefits package including personal development opportunities (CPD leave entitlement) and much more. Main Duties: Identify, and manage as appropriate, treatment plans for patients both at risk of developing a long-term condition and diagnosed chronic disease. Support the Infection Prevention Control (IPC) lead with the implementation of audits, training, support and evidence of compliance to the practice manager. Develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice. Develop, implement and embed health promotion and wellbeing programmes. Implement vaccination programmes for adults and children. About You: Registered Nurse with experience within a GP setting. Experience in chronic disease management inc. diabetes, hypertension, and respiratory conditions. Imms/Vacs trained, inc childhood imms.
