An exciting opportunity has risen for enthusiastic and motivated individuals to join our Reactive Emergency Assessment Community Team (REACT) as a senior Registered Nurse. The REACT team is a multi-disciplinary integrated community service within ESNEFT incorporating, Urgent Community Response service, OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours) and Virtual Ward pathways patients.

This role will be working on the crisis side of REACT. Working in the Urgent Community Response Team, which has aspects of working in partnership with East of England Ambulance Service and the Frailty Virtual Ward. Two very new and exciting government initiatives. Our team delivers excellent patient focused care to patients in Ipswich and the East Suffolk community. Could you be a valued key member of our team? The role involves travelling throughout East Suffolk delivering acute care in patient's home as a part of REACT. The successful candidate will be expected to work across the week including weekends and bank holidays. The hours will be 08.00-20.00 shifts. Pool cars are accessible for staff to use, and it is essential that post holder has valid driving licence. Main duties of the job

To work as part of the multidisciplinary team conducting face to face patient assessments, delivering planned treatment, and monitoring medical conditions for patients with acute deterioration, exacerbations of long-term conditions, palliative/ EOL care and rehabilitation needs. To be a key player in REACT with regards to triaging, prioritising patients with good clinical and diagnostics skills including venepuncture, cannulation, ECGs, catheter care, EOL care and line care. To work with the wider multidisciplinary integrated team in the community to prevent unnecessary admission to hospital. Excellent communication is required to ensure the best possible experience and outcomes for the crisis patients. This includes the ability to conduct excellent handovers, escalate and present patient cases within the VW MDT. To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users. About us
