The Role: The OT role is within the Integrated care team (LAP) which consists of Community Matrons, Mental Health Practitioners, Social Workers, Occupational Therapy Assistants, and Social Care Practitioners. The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital. Qualifications
What we are looking for: A qualified individual with some experience in the community or hospital setting A good understanding of IT with skills including MS office, work, and excel Must hold a UK driving license Disclosure and Barring Service Check
