We need a permanent, part-time Social Worker to work across Lancashire as part of our Adult Services Older Adults and Physical Disability Community Teams. As part of your exciting new role, you will be expected to undertake strength-based, outcome-focused assessments and support planning, ensuring that the residents of Lancashire receive the right amount of support. You will play a vital role in supporting people to access local services, maintain independence, promote health and wellbeing, and live independently. A key part of the role includes working alongside other agencies, professionals, and informal carers, providing a holistic service.
