Clinical Assessment Service Health Advisor

The Clinical Assessment Service supports and advises people 24/7 within North East Lincolnshire who present with acute and chronic illness, social care needs, those requiring unplanned care interventions and individuals in urgent situations. The Clinical Assessment Service is a diverse service receiving telephone and electronic referrals which are managed by a team of skilled Practitioners. The CAS supports both the public and professionals to provide the pivot for engagement with the multi-disciplinary teams and partner agencies. The CAS provides support to the locality NHS 111 provision. Please note that Care Plus Group are not currently registered to sponsor applicants from outside the EU and EEA. Main duties of the job

Permanent position with a rota pattern covering a 24-hour period, including weekends and Bank Holidays. Salary Range: £24,645 - £27,202 pro rata per annum, fixed salary point. Working under the direction of Seniors, you will be the first point of contact with patients, relatives, healthcare professionals, and other callers. You will undertake initial health and social care screening over the telephone and respond to referrals into the service, providing a professional front-line service to those that contact the Clinical Assessment Service. Essential Criteria

NVQ Level 3 in Customer Care or Health and Social Care or equivalent 2 years previous experience of working in a health and/or social care environment Ability to work under pressure, displaying patience, tact, and diplomacy Ability to assimilate information quickly and effectively An understanding of and respect for confidentiality in relation to all aspects of work Most importantly, you must be able to demonstrate your commitment to the values of Care Plus Group. Experience

Experience of using computerised systems Accurate keyboard and data entry skills Effective social skills gained from experience dealing with the public Experience of working within a confidential environment and/or handling sensitive or personal information Qualifications

NVQ Level 3 in Customer Care or Health and Social Care or equivalent Minimum of 3 GCSEs at Grade C or above or equivalent, including Maths and English Recognised IT or word processing qualification, e.g., RSA II Word Processing, CLAIT, ECDL or equivalent L4 Lead practitioner in care or similar Knowledge & Skills

Ability to assimilate information quickly and effectively Empathetic and sensitive approach Ability to communicate sensitively and effectively with callers Ability to overcome barriers to communication such as second language, behavioural difficulties, and cognitive deficits Ability to deal effectively with situations which may be hostile or aggressive Ability to effectively communicate with individuals/groups regarding difficult or complex matters Ability to work effectively as part of a team and able to use own initiative Ability to maintain accurate and legible records Act in a manner that supports the care, protection, and well-being of individuals Effective customer care skills Must demonstrate an understanding of confidentiality Awareness of safeguarding issues An understanding of the Clinical Assessment Service and its role within local health and social care service delivery An understanding of the implications of the General Data Protection Regulation and Freedom of Information Acts Disclosure and Barring Service Check
