MFT are currently seeking a passionateand skilled Senior Community Physiotherapist to join the team at the TraffordCommunity Rehabilitation team. This role involves conducting initial assessments,delivering specialist treatment plans for patients with complex medicalconditions, and working collaboratively with other healthcare professionals toensure optimal patient care. You will play a key role in evaluating andmodifying treatment plans, supporting service improvements, and mentoringjunior staff. Perform initial and specialist assessments toaid in medical diagnoses and formulate treatment plans. Plan, deliver, evaluate, and modify treatmentprograms for patients with diverse needs. Provide guidance to patients, carers, and juniorteam members. Manage a varied caseload, applying advancedclinical reasoning. Lead case conferences and participate in theservice's quality improvement initiatives. Mentor Band 5 Physiotherapists, Patient SupportWorkers, and students. About us

Qualifications

Degree in Physiotherapy Experience

Experience in managing complex cases in a community setting. Excellent communication skills for patient and team interactions. Disclosure and Barring Service Check
