The Practice Nurse (PN) will work as part of the clinical team providing personal medical services to our patients. You will work collaboratively within the general practice team to meet the needs of the patients including treatment, preventative care, screening & patient education. Responsibilities of our PN include: Provide nursing treatments to patients in participation with GPs or independently to agreed protocols. Assist in checking, ordering & maintaining stock levels in clinical room. Assess, plan, develop, implement & evaluate individual treatment plans for patients with a known long-term condition. Proactively identify, diagnose & manage patients who are at risk of developing a long-term condition (LTC). Work with other clinicians to diagnose, monitor, manage & treat LTC, including non-drug based treatments & in-line with national/local policies & practice needs. Undertake agreed procedures including Blood Test/pressure, height & weight-body mass index, venepuncture, ECG, peak flow readings, inhaler technique, spirometry, cytology & vaccinations. Prioritise health problems & intervene appropriately to assist the patient in complex, urgent & emergency care. Support & guide patients who are attending for family planning advice. Support & manage the needs of the women attending for cervical cytology. Any Administrative tasks as required. About us
Desborough & Hazlemere Surgery is a well-established practice serving 13,500 patients across two sites in High Wycombe, a large, diverse town in Buckinghamshire. You will become a key part of the team, already consisting of 8 GPs (including 2 Partners), experienced and organised Nursing Team (4 PNs & 2 HCAs), management and administrative teams as well as our wider Primary Care Network attached team including pharmacists, paramedics and social prescribers.
