Due to the increasing population of our surrounding communities, we are looking to recruit a keen and committed salaried Practice Nurse to join our friendly, supportive team. The Practice requires the successful candidate to work 15 to 20 hours per week, across both our High Wycombe and Hazlemere sites. We are very flexible to accommodate different working patterns for the right candidate. You will become a key part of the team, already consisting of 4 Practice Nurses and 2 Health Care Assistants. The Nursing Team is supported by experienced GPs, management and administrative teams as well as our wider PCN attached team including pharmacists, paramedics and social prescribers. We are a well-established practice serving 13,500 patients across two sites in High Wycombe, a large, diverse town in Buckinghamshire with excellent schools and amenities, minutes away from countryside with great transport links to London. We offer a contract with 5 weeks annual leave (plus Bank Holidays) and a NHS Pension. Informal chats/visits are welcomed and encouraged. The Practice Nurse (PN) will work as part of the clinical team providing personal medical services to our patients. You will work collaboratively within the general practice team to meet the needs of the patients including treatment, preventative care, screening & patient education. Responsibilities of our PN include: Provide nursing treatments to patients in participation with GPs or independently to agreed protocols. Assist in checking, ordering & maintaining stock levels in clinical room. Assess, plan, develop, implement & evaluate individual treatment plans for patients with a known long-term condition. Proactively identify, diagnose & manage patients who are at risk of developing a long-term condition (LTC). Work with other clinicians to diagnose, monitor, manage & treat LTC, including non-drug based treatments & in-line with national/local policies & practice needs. Undertake agreed procedures including Blood Test/pressure, height & weight-body mass index, venepuncture, ECG, peak flow readings, inhaler technique, spirometry, cytology & vaccinations. Prioritise health problems & intervene appropriately to assist the patient in complex, urgent & emergency care. Support & guide patients who are attending for family planning advice. Support & manage the needs of the women attending for cervical cytology. Any Administrative tasks as required. About us

Desborough & Hazlemere Surgery is a well-established practice serving 13,500 patients across two sites in High Wycombe, a large, diverse town in Buckinghamshire. You will become a key part of the team, already consisting of 8 GPs (including 2 Partners), experienced and organised Nursing Team (4 PNs & 2 HCAs), management and administrative teams as well as our wider Primary Care Network attached team including pharmacists, paramedics and social prescribers. There is a friendly atmosphere established at both our sites, a long we a 'can do' work ethic. There is a loyal and well established team in place, with a very low staff turnover rate. Experience of working in accordance with the NMC - The Code: Standards of conduct, performance and ethics for nurses and midwives Experience of working in a Primary Care Environment Experience of Microsoft Office software Experience of dealing with the public/patients Ability to follow clinical policies and procedures Experience of QOF and clinical audits Experience of Clinical systems such as EMIS, DocMan, AccuRX and ICE Commitment to the local area Awareness of Equality and Diversity principles Understanding of confidentiality and the Data Protection Act Leadership and management experience Minimum of one year experience working as a Practice Nurse in the UK Qualifications

Registered General Nurse Recognised Qualification to Diploma Level or above Evidence of continued personal and professional development Appropriate safeguarding training Registered on the National Medical Performers List UK work permit (if required) Knowledge/Skills

Competent in basic nursing duties required for the post Specific nursing skills / qualifications Disclosure and Barring Service Check
