Mildenhall Integrated Neighbourhood Team Permanent, Part-time opportunity We are looking to recruit a Band 6 Occupational Therapist to work in a friendly and well-led community health and social care team. We are developing the Integrated Care Model and therefore have close, co-located working relationships with our Social Care colleagues. Key Responsibilities

Holistic assessment and treatment of people in their own homes. Work autonomously in the community. Assist people with long-term conditions, palliative conditions, and rehabilitation needs to achieve quality of life and independence. Collaborate with health care professionals and our wider multidisciplinary team to provide an integrated service. Commit to providing a high standard of care and quality at all times. Main Duties of the Job

The successful candidate will contribute to the delivery of high standards of health care to patients within their own homes, residential care homes, and clinics by performing holistic assessments and delivering therapy interventions. Formulate individual specialist management/treatment plans using clinical reasoning skills and a wide range of treatment skills. Support junior staff in managing their caseloads using evidence-based, client-centered principles. Work within the integrated team to facilitate early discharge from hospital or prevent unnecessary admissions. Collaborate with all health care professionals and agencies to provide seamless, integrated services. Assist the Team Manager and Clinical Specialist OT in developing therapy services. Identify training needs through the appraisal process. Support the Practice Placement Education of students. Person Specification

Education & Qualification

Diploma or Degree in Occupational Therapy/Physiotherapy Health Professions Council Registration Documented evidence of Continuing Professional Development Experience & Knowledge

Experience supervising staff and students. Knowledge of Health & Safety legislation. Experience in rehabilitation of adults with physical disabilities. Experience working with primary and community services. Skills & Abilities

High level of communication skills. Ability to reflect on and appraise own practice. Standard computer skills including Microsoft Office. Effective leadership skills. Personal Qualities
