KEY RESULT AREAS: To work collaboratively with MDT members tocontribute to a flexible and effective interdisciplinary team approach toclinical care. Communicate face to face with clients on adaily basis, some of which display challenging behaviour. To effectively establish a therapeuticrelationship with service users and their relatives/carers. To assess capacity and obtain informedconsent from service users and work within a legal framework with thoseclients who lack capacity to consent to treatment. In conjunction with colleagues toundertake, record, review and work within individual risk assessment andmanagement plans in line with service and Trust policy. To provide mental health assessment ofclients with acute mental health needs, clearly assessing and managingrisks. To lead joint reviews for patients withcomplex needs to plan transfer of care. To review patients for transfer to onwardsmental health support as per MDT plans. To ensure service users are involved in theplanning and prioritisation of their care plans wherever possible. To adapt practice to meet individualservice users circumstances, including regard for cultural and linguisticdifferences. To work collaboratively with service users,staff, and relatives to identify goals and determine and deliver anappropriate intervention plan which aims to maximise engagement andperformance whilst balancing risk, and promoting choice and independence. To monitor, evaluate and modifyinterventions in order to meet needs and to measure progress and increaseefficacy for service user outcomes. To demonstrate and apply understanding ofimpairments to mental well being and their impact on function, includingstrategies to manage impairments and potential risks. In conjunction with others to identifyclients needs for additional care provision / housing/specialistplacement/further assessment /referral on to other agencies. To act as a source of clinical referencefor other colleagues regarding the service users, providing advice to enableand improve service/care provision. To attend and actively participate inrelevant clinical meetings/forums. RESPONSIBILITY: You will be working across twofunctions-Rapid Response and Home Treatment Team providing assessments andinterventions until the crisis is resolved. To screen and work with all newly referredservice users experiencing acute mental health crisis providing assessment orshort-term interventions until crisis is resolved. To provide short-term specific psychosocialinterventions for those who would benefit from crisis interventions To ensure documentation is reviewed andupdated in line with quality improvement standards To shift co-ordinate lead the shift,allocate work to the wider MDT and prioritise workloads. To contribute to the planning andimplementation of induction, training and education of students and staff. To contribute to the delivery ofspecialist/ professional training packages of in-service training and staffdevelopment when necessary. To fulfil the role of mentor/practiceeducator, maintaining own update training and ensuring responsibility forregular placement provision and ensuring supervision and written assessmentof students whilst on placement. To assist the band 7 senior practitioner byidentifying personal and professional training needs in order toappropriately plan for future training and development. To provide effective supervision, support,and guidance for support /STR/ peer workers and any work delegated to them. Responsible for the delivery of individualcare plans for a caseload Expected to implement policies as directedby the trust/directorate for the department, ensuring awareness throughoutthe department. Participate in leading servicedevelopments. Knowledge and expertise in the applicationof relevant professional theoretical models, and approaches relevant toclient group. Communicate complex, sensitive andemotional information using a variety of mediums; being able to adaptmethods/style/approach as appropriate. Awareness of clinical governance and itsrelationship to clinical effectiveness and patient safety and patientexperience. To have effective organisational /planningand decision-making skills to manage workload and shift co-ordinate Good knowledge of IT skills ENVIRONMENT: Based in the CRHT team Shared office space. Full drivers license with businessinsurance to allow ability to move between locations in a timely manner. Conduct home visits with all patientsunless risks determine to meet at a community site May be redeployed to work within acuteinpatient wards which may require individual to Restrain patients usingapproved techniques as part of trust contingency planning. Counsels clients with mental health needson a daily basis. To be able to work a 24hr shift patternincluding day shifts, nights shifts and weekend shifts JOB SUMMARY: Mostchallenging parts of the job: Demonstrates ability to work as a teammember as well as alone making autonomous decisions in a fast-pacedenvironment. Supporting Band 7 to manage a case load ofservice users who have complex acute/crisis needs Balancing all aspects of the job role. Being flexible and responsive to competingdemands COMMUNICATIONS AND WORKING RELATIONSHIPS: To apply a comprehensive range ofcommunication skills with service users, relatives, team members, and otherinternal and external stakeholders, adapting communication as necessary. To effectively liaise with clients, family,carers, advocates and health and social care professionals regarding clientsassessment, treatment and care to ensure continuity of care and delivery of acoordinated service as a member of the MDT and implement core decisions withregard to urgent clinical issues. To ensure all communication adheres toTrust policy and Caldicott / data protection principles. To provide accurate, timely reports anddischarge summaries. To deal with e-mail correspondence in atimely manner. To explain the professional role in CRHT tovisitors, students, volunteers etc. To monitor the record keeping of junior /non-qualified staff and students where appropriate. To link with special interest groups bothinternal and external to the Trust to share good practice To act as a representative of the serviceas required. Person Specification

Qualifications

Degree or equivalent in Occupational Therapy Experience

Experience working in mental health service Disclosure and Barring Service Check
