Rotherham, in line with national requirements, provides extended access to routine general practice appointments on weekday evenings (6.30pm - 8.30pm) and weekends (8am - 1pm). This is provided from five general practices acting as hubs in North, South, and Central locations in the Borough. Appointments are available for patients registered with any of the 30 general practices in Rotherham with a range of clinicians, to offer the convenience of a routine appointment out of core hours. A GP will be present in the practice, providing medical cover and support for clinicians on all shifts. Main duties of the job

The post holder is responsible for the delivery of routine healthcare provision to Rotherham's general practice patients. Supported by senior Clinicians within the practice, they will deliver care within the boundaries of their role, focusing upon health promotion, prevention of ill health, and the maintenance and monitoring of long-term conditions and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and work under the supervision of the registered nurse and senior clinicians. The organisation currently delivers extended access primary care services in Rotherham, providing evening and weekend appointments to patients out of core hours to ensure patients have access to GP, Nursing, HCA, and pharmacist appointments. The organisation is supporting the development of primary care networks and manages delivery of training and development to large cohorts of the general practice workforce. Our Nursing team champions new models of development and training to upskill the existing workforce at all levels, in addition to promoting new roles and supporting training schemes in general practice. Job responsibilities

Assist nurses in planning, developing, implementing, and evaluating programmes to promote health and well-being, and prevent adverse effects on health and well-being. Assist in helping to identify appropriate care and treatment plans for patients at risk of developing a long-term condition. Know how to act appropriately in emergency situations and in the deteriorating patient. Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care. Deliver opportunistic health promotion. Care for patients presenting with uncomplicated wounds that have been assessed initially by an RGN and have a care plan to follow. Venepuncture and sample taking. Urinalysis. Ear Care (dependent on training). Setting up equipment for procedures. Decontamination and infection control measures. Blood pressure, pulse, and other observations as necessary. Height and weight and checking of BMI. Person Specification

Qualifications

Mandatory training completed annually. Evidenced qualifications in skills and updates and evidence provided of care certificate/NVQs or equivalent. Knowledge

Awareness of the needs of patients with long-term conditions. Aware of accountability of own role. Knowledge of health promotion strategies. Awareness of clinical governance issues in primary care. Knowledge of patient group directives and patient-specific directives and associated policy. Basic IT skills. Use of clinical systems. Ability to identify determinants on health in the local area. Knowledge of public health issues in the local area. Awareness of local and national health policy. Awareness of issues within the wider health economy. Skills
