This is a part-time, permanent post of 22.2 hours per week, with a shift pattern of 9am-5pm, 3 days a week. Please note the salary is Pro Rata for part time hours. Role Overview

The post holder will be a Registered Nurse required for West Dunbartonshire Community Children’s Home/ASL School Nursing Team (CCNT) as part of Specialist Children’s Services NHS GG&C. The team delivers nursing care to children and young people 0-19 years in the community and in Kilpatrick (Additional Support for Learning) School with acute and complex healthcare needs. Responsibilities

Carry out Nurse Led Assessments within the School and participate in delivering the immunisation programmes. Conduct home visits and work within the paediatric acute and community clinics and nurse led phlebotomy clinics at Acorn Centre. Perform sleep work/melatonin reviews for the Disability Service. Work independently making appropriate clinical judgements and as part of a multi-disciplinary team, promoting integrated nursing approaches. Participate in assessment/review of children and young people including those with additional support needs. Develop, implement and evaluate care plans, providing nursing care as necessary. Promote the principles of self-care to children, young people, families and carers and empower them to access and use health resources in the community. Function as an independent autonomous practitioner and lead/deputise in the absence of the Nurse Team Lead. Be accountable for case management ensuring that service provision is effective, efficient and evidence-based.
