Hours:

35 hours per week

Annual Leave:

28 days annual leave (plus bank holidays and wellbeing day)

Contract:

Four-year fixed term expiring on 31st March 2029 (satisfactory to six-month probationary review, with a possible extension)

Job Description You will lead and coordinate the development of Beyond Empowers Community Social Care (Community Outreach) offer.

This will initially involve directly supporting and creating groups of disabled people to access health, wellbeing and social activities of interest in the community.

Alongside this, you will grow the offer to enable the recruitment and appointment of others to provide direct support to those accessing the offer, allowing you to focus solely on the coordination of the offer long-term.

You will work closely with key stakeholders; such as current and potential users of the offer, parent and carer forums, social and health care, and community organisations, to raise awareness, generate referrals, and support the offers growth and development.

You will be able to engage and communicate easily at all levels, and be adaptable and creative in your approach. You will be able to work alone, as well as part of a small team, with the ability to deal with difficult situations. With a record of personal development and learning, you will be able to travel easily around Salford and surrounding areas, to work flexibly according to the needs of those you support and the organisation.

At times, the Community Social Care Coordinator role will be challenging and require confidence and resilience. However, for the right person, as a new role in a new area of work, it offers the opportunity for a person to really make it their own, while supporting a more diverse and person-centred community social care landscape to enable people greater choice, variety and outcomes.
