Poppy Ward at Ellesmere Port Hospital is a 19 bedded general rehabilitation unit under the care of Hospital at Home with facilities for community patients to step up for care / rehabilitation. We are looking for a dynamic and enthusiastic Registered Nurse to join our joint nursing and therapy team.
The successful candidate will continue to develop skills of clinical reasoning and supervision in a supportive, caring and friendly environment, ensuring the delivery of high-quality patient care is maintained. You will enjoy the excellent ethos of multidisciplinary working to ensure effective and efficient patient flow between the Countess of Chester Hospital and Ellesmere Port Hospital.

Main duties of the job
Be accountable for performing regular assessments on patient needs, to plan, implement and evaluate care delivery and be adaptable to changing health care requirements. You will be responsible for establishing and maintaining effective communication with patients, relatives and carers to always ensure their involvement in the decisions about treatment, planning and the delivery of their care. You will take responsibility for working collaboratively with your team, other professionals and agencies to always ensure the patient's needs are met, especially in relation to their ongoing health care needs, their transfer and their discharge planning.
