CWP (Tier 4 CAMHS) provides mental health services for children & young people with emotional, psychological and mental health related difficulties. The services are provided through assessment for intensive support delivered through a residential inpatient setting or a home based treatment approach. A whole systems approach is utilised in assessing and offering interventions and support. To work as part of a team on a rota basis including unsocial hours. To promote and uphold CWP Trust Values. Be responsible for undertaking the assessment of young people referred to Tier 4. To undertake all assessment related activities from screening and triaging referrals, coordinating the assessment, undertaking the assessment, recording the findings and outcome of assessment on the patient record, communicating the outcome of the assessment with all the relevant stakeholders and arranging any interventions offered. Professional Registration with recognised National Body (HCPC, NMC, Social Work England) Nationally recognised post-qualification, continuous professional development (CPD) related to the core profession (Social Work, Nurse, Allied Health Professional). Significant post qualification experience of working within a CYPMHS or Child Mental Health related field.
