An exciting opportunity has arisen for 4 x PCN CYP Mental Health Practitioners to work into our local Primary Care Networks (PCN) GP practices to support the needs of our local population living with mental health difficulties (children and young people). This is a new and innovative service supporting young people in primary care settings and working in a collaborative and multi-agency way to meet the needs of patients with a range of mental health needs. GP practices have begun working together alongside community, mental health, social care, pharmacy, hospital, and voluntary services in the local area to form PCNs. PCNs signal a significant change in the way practices interact not only with each other but the wider health and social care system. This will enable care to be delivered to a defined patient population, in a different way to meet the needs of that population. This will be focused on prevention and personalization of care, making best use of resources collectively. There are 4 posts across the Teesside area (Middlesbrough, Redcar, Stockton & Hartlepool) and these will be discussed and allocated following the interview. Main Duties of the Job

The post will sit within the GP practices and will offer assessment, treatment/brief interventions, and review for children and young people. The role will be offering specialist mental health clinical care and will also offer networking expertise and navigation to a range of specialist mental health supports to ensure each child/young person once assessed receives the best possible care in the right place at the right time by the right supporting service. Qualifications

Registration with relevant professional body (e.g. NMC, GMC, HCPC, Social Work-England) For nursing professions - Recognised Practice Supervisor and/or Practice Assessor, clinical educator, or equivalent AND Trained in providing clinical supervision For other professions - evidence of training in clinical supervision of other staff and able to demonstrate competence in this area of practice in clinical supervision of other staff and/or students/trainees All professions - Postgraduate diploma (or above) in a subject relevant to the area of practice or be willing to work towards and achieve within 2 years. Within the first year of being in post hold a recognised advanced clinical qualification or Masters level credits e.g., Intermediate Systemic Family Therapy/Post Grad diploma in CBT/British Isles Dialectical Behaviour Therapy/NMP e.g. dysphagia, posture, autism assessment, rebound, social prescribing, sensory processing, Approved Mental Health Professional, Practice Educator (level 3 Independent Prescriber) And have equivalent clinical experience to that of a Post Grad Diploma/master's level such as working as a band 6 (or above) in a relevant clinical area with significant experience, together with evidence of relevant CPD Experience

Substantial clinical experience of working in relevant specialist area in collaboration with service users, families, and relevant others or transferable experience of similar settings and willing to gain knowledge within the first 12 months. Experience of assessing and managing risk (harm to self, others, and safeguarding) Substantial experience of working in a multi-disciplinary team Involvement in quality improvement activities Knowledge

Extensive knowledge of current issues relating to health needs of patients in the field of practice, regionally and nationally Understanding and working knowledge of the legal framework and statutory obligations under the Mental Health Act, Mental Capacity Act Detailed understanding of Safeguarding and its application in practice Clinical Risk Assessment and Management and its application in practice Clinical Governance and its application in practice Working knowledge of clinical medicines management Understanding of the physical healthcare needs and impact of patients with a LD or mental health problem. Community Transformation Framework or iThrive Framework and its application in practice Knowledge of Stepped Care model Care Programme Approach and its application in practice Research and development methodology Understanding of psychological models of care and treatment Demonstrable knowledge and experience of delivering psycho-social interventions and approaches Contemporaneous knowledge and understanding of recent health and social care policy guidance and implications on practice Disclosure and Barring Service Check
