Being a Care Home Liaison Mental Health Nurse in the NHS is a versatile and impactful role that spans across various healthcare settings, from hospitals to community clinics. It involves providing comprehensive care to patients of all ages and backgrounds, addressing both acute medical needs and long-term health management. The flexibility in scheduling allows general nurses to balance their professional responsibilities with personal commitments, ensuring they can deliver compassionate and effective care while maintaining a healthy work-life balance. What you can add: To work as a member of a Multi-Disciplinary Team reporting back to colleagues, both verbally and in writing and seeking advice, guidance and support where necessary. To provide each patient for whom they are a Lead Professional with an agreed care plan, containing review dates, in accordance with Community Mental Health Nurse and Care Programme Approach standards. To actively seek to contribute to setting standards of care, to generate and implement quality initiatives. To promote compliance with medication and other treatments prescribed, noting and reporting non-compliance and side effects as appropriate. To develop and maintain healthy partnerships with all stakeholders including statutory, third sector and voluntary agencies. To advise patients, carers and significant others of the available resources. Referring assessed need to those agencies best able to meet these in the community and facilitate use of these resources. To understand the requirements of the Mental Health Act 1983, Mental Capacity Act 2005, Care Act 2014, Care Programme Approach, Deprivation of Liberty safeguards as it applies to a psychiatric liaison service. The role will involve, but not be limited to you performing the above. What you'll need: A minimum of 6 months paid experience in a mental health hospital environment within the last 3 years. Adult Mental Health Practice. Community Mental Health Practice. Older Peoples Mental Health Practice. Hospital/Care Home Liaison. Working with physical healthcare services. Specific Skills: Evidence of keeping up to date. Access to transport and the ability to travel within the required geographical area. Experience of being supervisor (Clinical/Managerial). ENB998 (Teaching) or equivalent. Risk assessment training. Clinical Intervention skill training. *Depending on the Trust, you may require variable additional certification and skills.
