We are seeking a talented, patient-focused clinician to join our fantastic team working in partnership with Peterborough City Hospital. As a Registered Nurse, you will deliver gold standard, one-to-one, holistic care in the comfort of the patient's home. You will work as part of a multi-disciplinary team delivering one-to-one care in line with the patient's needs, whilst maintaining professional standards and following the relevant care plan. Deliver effective and high-quality nursing care to patients in a home environment to facilitate early supported transfer from hospital or to prevent unnecessary admission. You will be building lasting relationships with patients, stakeholders, and patient support networks to aid and support patients' treatment and recovery. Accurately document all patient care and clinical observations to provide real-time data to assist with long-term goals of the business and the patient. Knowledge

Delivering patient care in patients' own homes according to an agreed programme of care and care plan. Conducting an accurate holistic assessment of patients' needs in accordance with the approved model of care. Safely administering a range of therapeutic interventions relevant to NHS project and training as per agreed clinical pathways within the scope of practice. Assisting patients in their activities of daily living and supporting multi-disciplinary goals of optimising patients' level of independence. Conducting roles and responsibilities in accordance with NMC Code of Practice and HAH policy, procedures, and protocols. Having responsibility for the health, safety, and welfare of self and others and complying at all times with requirements of Health and Safety regulations, including reporting any accidents or incidents using relevant documentation. Being fully aware of and understanding through relevant training provided by Healthcare at Home, the duties and responsibilities pertaining to the safeguarding of all patients, both adults and children. Being fully aware of the principles of safeguarding as they apply to all vulnerable adults and children in relation to the specific role, which will include recognising the types and signs of abuse and neglect and ensuring that any relevant line manager is made aware and kept fully informed of any concerns which the worker may have in relation to safeguarding adults and/or child protection. Skills

RGN part 1 NMC Register (RSCN where appropriate). Experience in an acute care setting post-registration, including some time taking independent clinical decisions and supervising the work of others. Strong understanding of therapies and drugs relevant to service offerings, including NHS knowledge relating to the management of patients with treated conditions. Pays close attention to detail and follows rules and protocols, quality-oriented. Strong communication skills, including actively listening, communicating sensitively, putting people at ease, and making technical details easy to understand. Experience in the care of IV devices and administration of IV therapies. Experience of undertaking holistic clinical assessments. Disclosure and Barring Service Check

Salary: £32,240 to £37,960 a year plus a company car or car allowance of £4887.50.
