The job role includes the following:

To provide specialist nursing support to people at risk of breakdown or admission to mental health hospital for assessment and treatment.

To provide specialist nursing support to clients at risk of placement breakdown due to mental health difficulties.

As part of a team to review Section 117 aftercare plans for clients known to the ICB residing within the NWL area.

a clinician who keeps the client at the centre of their practice.

a dedicated motivated individual who has the experience to work independently within a multidisciplinary team

Main duties of the job The main duties of the job are supporting clients with learning disabilities and mental health difficulties in the community who are at risk of admission or placement breakdown. You will be working closely with the client and their support network.

You will be expected to complete assessments relating to mental health and learning disabilities including identifying strengths and risks and developing care plans reflective of the clients needs.

As part of a team you will manage a small caseload of clients who require a yearly review of their section 117 aftercare plans. The aftercare of these clients is held by the ICB and the clients reside within the North West London area.

You will work closely with the Consultant Psychiatrist providing mental health support and liaise with the wider Community Learning Disabilities team including nurses, psychologists, speech and language therapists and occupational therapists.

We are looking for someone who has experience:

putting the client at the centre of their practice

working with people who have a learning disability and/or

working within mental health

understanding of the Dynamic Support Register

Understanding of Section 117 after care review process

About us The LD Psychiatry service works across three boroughs offering the psychiatry input into the Tri-Borough LD Community Team. This post offers the flexibility to be based in Hammersmith & Fulham, Kensington & Chelsea or Westminster.

The role requires travel around the three boroughs. We are looking for a clinician who is able to work independently and to organise their time effectively.

You would receive clinical supervision from the Trust Consultant Nurse for Learning Disabilities and be part of the active Learning disabilities and Autism Clinical network. Management supervision would be provided by the LD Psychiatry service manager.

Job responsibilities Your role will be to provide Health Case Management/named worker specifically for ICB responsible clients who are at risk of placement breakdown or admission and/or on the DSR.

The role of case manager is multidimensional, requiring a unique blend of clinical knowledge, people skills and administrative abilities.

The primary function of this role is to support people who are at risk of placement breakdown or admission to mental health hospital for assessment and treatment or face placement breakdown due to mental illness / behaviours of concern.

You will be part of a team responsible for reviewing S.117 aftercare plans, alongside other involved services, of Tri-Borough patients, including those in placements outside of the Tri-Borough but within the NWL Commissioning area. This involves following a review process and creating aftercare plans to support the client.

You will work within the multi-disciplinary team structure of the Tri-Borough Community LD Services, providing specialist professional advice and expertise in learning disabilities for this cohort of patients, this can include supporting clients when medication is initiated within case load numbers. The community LD service consists of nursing, psychology, speech and language therapy and occupational therapy.

The post-holder will establish and maintain a high standard of care, ensuring that the quality of care provided is continually informed by the best evidence-based practice.

Person Specification Education

Registered NMC RNLD and/or RMN

Evidence of post-registration qualification

Educated to degree/Diploma level

Previous Experience

Extensive post registration, experience with experience of working with a learning disability service

Highly specialist knowledge of the user group in the learning disabilities agenda

Experience of managing a clinical staff team

Experience of multi-professional collaboration, partnership working with other statutory and voluntary agencies

Experience of setting, monitoring and evaluating standards of care in community settings

Experience of working in a multi-cultural environment and knowledge of methods to develop culturally sensitive practice

Experience of cultural awareness/diversity training and the ability to implement this

Able to evidence change practice, culture and outcomes of nursing teams

Physical Intervention trained

Experience of managing service users who exhibit challenging and high risk behaviours, that require advanced interventions to enable effective de-escalation

Experience of using creative and sophisticated interventions with service users in distress

Participation in audit cycle

Project Management

Knowledge

Knowledge/understanding of the physical and mental needs of service users being cared for within this service

Knowledge of risk assessments and audit/research, ability to implement and teach others

Knowledge of clinical supervision process and the ability to train others

Knowledge of current professional developments in nursing

Understanding of CQC processes and requirements

Understanding of research and audit methodology.

Understanding of budgeting and financial control

Understanding of de-escalation techniques

Basic knowledge of IT skills

Ability to provide leadership to staff from a variety of mental health professions

Ability to motivate staff to provide a quality mental health service to users and carers alike.

Ability to prioritise staff resources to target those areas of greatest need

Ability to manage devolved budgets.

Proven ability to provide clinical supervision, to challenge and commend as required

Ability to evaluate services.

Exposure to odours, body fluids and verbal aggression

Sound written and verbal skills

Proven ability in clear and concise report writing.

Ability to work under pressure, meet deadlines and self-regulate.
