The community staff nurse role would include holistic assessment of patient care needs and development, implementation and evaluation of evidence care.

Our priorities within the team focus on: Caring with humanity, dignity, kindness and compassion Providing safe, effective, harm-free care Listening to patients and carers and improving the way we work Working within the community resource team and industry partners for the benefit of patients and the population A questioning, analytical, empowered Nursing and Midwifery workforce

The post holder will participate in the delivery of skilled, autonomous nursing care to patients within their own home. Duties include palliative care support and assessment, care at the end of life, supporting and assessing individuals and carers with long-term health conditions, promotion of self-management skills and wound care.

Main duties of the job

The post holder will be responsible for autonomous evidence-based safe delivery of care and treatments in patient homes within the community, in line with competency and training record and organisational policies, procedures and guidelines. Assessing, planning, implementing and evaluating care without direct supervision.

To work in partnership with patients and carers to offer support, advice, and promote informed choice. Working and collaborating with the multidisciplinary team to provide gold standard care. Support the role of the community nurse team leader. The ability to speak Welsh is desirable for this post. English and/or Welsh speakers are equally welcome to apply.
