We are offering a flexible and varied role, training, working as part of a dynamic team, hybrid working, and community-based interventions and prevention. Main Duties of the Job

We are looking for an enthusiastic and motivated person to join our team in Knowsley. The successful candidate will deliver stop smoking advice and support to service users in a variety of settings including face to face, telephone, and community promotion. Experience

Demonstrable experience working with community groups, voluntary organisations in some capacity or working in a GP practice. Knowledge and experience of IT systems including the ability to use Microsoft Office: Outlook, Excel, Word, and PowerPoint. Experience of data collection and involvement in service monitoring and evaluation. Experience of working and supporting individuals on a one-to-one basis. Experience and ability to identify risk and assess/manage risk when working with individuals. Experience of building relationships and partnerships. Experience of working collaboratively with other healthcare and non-healthcare professionals to meet patient needs. Qualifications

Demonstrable commitment to professional and personal development. Willingness to complete the NCSCT CHCP Health Trainer training course. Level 3 or working towards an equivalent qualification in a relevant subject area and/or significant experience of working within a healthcare setting. Training completed in motivational coaching/interviewing or equivalent experience. Knowledge

An understanding of the role of voluntary and community sector organisations and the contributions they make to the health and wellbeing of individuals. Knowledge of local communities, including resources and assets, VCSE, and community services in the locality as well as a commitment to work collaboratively with all local agencies. Knowledge of factors influencing behaviour and behaviour change approaches. An understanding of the factors that can impact on an individual's health and wellbeing. Understanding of the wider determinants of health, including social, economic, and environmental factors and their impact on communities, individuals, their families, and carers. An understanding of community development and asset-based approaches, building on existing community and personal assets. An understanding of the importance of accurate record keeping and knowledge of information governance requirements. Knowledge of health and safety requirements in relation to processes for risk assessment in relation to venues and home visits. Knowledge of the personalised care approach. An understanding of health and social care infrastructure and how services are provided to the local population. Have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies when what the person needs is beyond the scope of the link worker role. Have a specialist knowledge of one of the wider determinants of health (for example, mental health, housing, debt/finance, Smokefree) and commitment to continually develop further knowledge, skills, and expertise within this area. An understanding of the barriers and challenges that people may face to become Smokefree and how to overcome the challenges in connecting to and accessing local activities and support.
