Ranworth Primary Care Network has an exciting opportunity for a qualified clinical pharmacist to join our PCN on a full-time, permanent basis. The role is covering the Ranworth PCN, which is made up of 4 practices and around 31,000 patients in Clacton on Sea and Frinton on Sea. This position is to commence in April 2025. The post-holder will work across our multidisciplinary team to provide expertise in clinical medicines management, medication reviews, and repeat prescription authorisation. The post-holder will be supported to develop their role to become a non-medical prescriber if that qualification is not already held. Main duties of the job

Review the ongoing need for medicine of patients facing long-term conditions, monitoring needs and opportunities to support patients with their medicines ensuring they get the best use of their medicines (i.e., medicines optimisation). Make appropriate recommendations to senior pharmacists or GPs for medicine improvement. Work with care home staff to improve the safety of medicines ordering and administration. Attend and refer patients to multidisciplinary case conferences. Management of common/minor/self-limiting ailments. Provide a telephone help line for patients with questions, queries, and concerns about their medicines. Produce and implement a repeat prescribing policy within each PCN practice. About us

Ranworth Primary Care Network is comprised of Caradoc Surgery, Ranworth Surgery in Clacton-on-Sea, Green Elms Health Centre in Jaywick, and Kennedy Way Medical Centre in Holland on Sea. We have a diverse patient population of around 32,000, and all practices work on SystmOne. Person Specification

Qualifications

GPhC registered Pharmacist. Minor ailments certificate. MUR and repeat dispensing certification. Experience

Experience in managing pharmacy services in primary care. Broad knowledge of general practice. Disclosure and Barring Service Check
