We are looking for an enthusiastic andmotivated Band 7 Clinical or Band 8a Advanced Clinical Pharmacist (depending onexperience) to be part of our pharmacy team covering a team members maternity leave. This post is a one year fixedterm contract. Our team provideclinical pharmacy and medicines supply services to Mental Health, LearningDisability and Community services across the Trust. Our pharmacists andpharmacy technicians are valued members of the multidisciplinary teams and theservice is currently in the process of implementing a strategy to expand ourclinical services and deliver a high quality medicines optimisation serviceacross all trust services. We have also recently introduced electronicprescribing and medicines administration system across the Trust The postholder will be responsible forthe delivery of day to day medicines optimisation and clinical pharmacyservices to patients within our care in the Wakefield / Kirklees localities. Ifthe successful candidate meets the requirements for the Advanced ClinicalPharmacist (8a) role then they will have the additional responsibility of linemanagement and overseeing a clinical pharmacy service to a designated area. Main duties of the job

Thepost will be based across Fieldhead hospital in Wakefield and The PriestleyUnit in Dewsbury (but the postholder may occasionally be asked to work at otherTrust sites). Thepost holder will be responsible for providing clinical pharmacy services to designatedinpatient units / crisis team / community teams with involvement in thedelivery of patient care, contributing to MDT meetings and delivering educationto patients and professionals. Thesuccessful candidate will also support the departmental and Trustwidegovernance agenda, providing clinical input into audit and service developmentprojects. They will support the day to day operation of the service, providingtraining and staff supervision as necessary. We aim to provide a pharmacy and medicinesoptimisation service which works collaboratively and compassionately with allservices and service users to provide integrated, high quality, person centeredpharmacy care and services to all. Provision of Clinical Pharmacyservice to allocated teams Provision of high qualitypatient focused pharmaceutical care. Provison of professionalresponsibilities of a pharmacist supervising the dispensing of prescriptions atFieldhead Pharmacy whilst fulfilling the dispensary commitment. Participation in the Saturday,Bank Holiday and On-call Rotas In conjunction with the pharmacyteam, develop and implement safe and effective policies and procedures in linewith New Ways of Working To provide management toallocated staff including appraisal , development and clinical supervision To be involved in thedevelopment of new services as allocated by pharmacy management team Develop and participate inTrust-wide medicines management audits and attend allocated governance groupsto represent pharmacy. To be responsible for the clinicalpharmacy and prescription monitoring at allocated clinical teams . Be aspecialist medication referral point for these teams. Medicines reconciliation targetsfor inpatient area Ensure all service users haveaccess to clinical pharmacy services Screening for all service usersadmitted to intensive home based treatment services. Develop and contribute to patientfocused services to allocated teams , including service user groups. Provide appropriate and timelyverbal and written information on all aspects of medication usage toservice users, carers and healthcare professionals within SWYPFT. Participate in in-house trainingfor pharmacy staff and healthcare professionals , both attending anddelivering training. Participate in the training ofpre-registration pharmacy graduates and mental health pharmacy diplomastudents. Participate in the development ofthe medicines management clinical audit programme in particular the POMHaudits and ensure roll out to allocated units. Participate in research anddevelopment as required for the development of the clinical pharmacyservices. To cover for other seniorpharmacists in their absence. If required due to sickness orother absence to work in other localities of the organisation. To carry out CPD in line with the GPhCrequirements. To maintain and demonstrate a goodclinical pharmacist knowledge. To work within the policies andprocedures of the South West Yorkshire Partnership NHS Foundation Trustand also to ensure compliance with the pharmacy policies and proceduresfrom other organisations when working in partnership. Qualifications

Band 7: GPhC registration as a Pharmacist Band 8A: Current registration with GPhC Membership of CMHP Post graduate qualification in clinical /mental health pharmacy or equivalent experience Membership of the Royal Pharmaceutical Society (RPS) Band 7: Membership of CMHP Membership of RPS Band 8A: Clinical or psychiatric diploma or relevant experience Accredited member of the College of Mental health Pharmacists.(CMHP) or RPS Faculty Postgraduate Certificate and Diploma in Psychiatric Pharmacy Supplementary prescribing qualification Physical Attributes

Band 7: Completed pre-registration training Substantial demonstrable experience of working post registration as a pharmacist Band 8A: Demonstrable substantial post registration as a pharmacist including experience in mental health.
