A flexible approach to practice, excellent communication skills, the ability to work autonomously and a qualified driver are all essential for this post. The service provides complex care to patients with heart failure within the community setting. The post-holder will be joining a multi-disciplinary team working with other Heart Failure Specialist Nurses, Cardiologists, and the wider Brent Integrated service. The position involves working closely with the primary and secondary care teams, to provide specialist heart failure care and expertise to patients diagnosed with heart failure, and to ensure that patients receive seamless care in hospital, at home and in the outpatient setting. As well as the clinical delivery, you will be required to lead on various service development projects such as audits, patient stories, and patient engagement. In addition, you will be contributing to up-skilling other health care professionals to provide a high standard of care. About us

Registered Nurse Current active registration with NMC Degree or relevant post registration experience Qualification in independent nurse prescribing or willing to undertake qualification Hold or working towards qualification in physical assessment Chronic disease management course/ experience.
