Responsibilities: Conduct comprehensive mental health assessments for older adults, considering cognitive, emotional, social, and physical health factors. Develop individualised care plans to address mental health needs, working in collaboration with patients, families, and other professionals. Participate in MDT meetings to review care plans and discuss patient progress. Respond to
mental health crises , providing urgent support to older adults experiencing acute distress or deterioration in mental health. Support patients with dementia and their families in managing symptoms and planning for future care needs. Qualifications and Requirements: Hold a degree in mental health nursing.
