An exciting opportunity has arisen for a qualified Band 6 Social Worker to join our dynamic multi-disciplinary Specialist Mental Health Service for Older adults serving the needs of older people within the population East Hertfordshire covering Broxbourne Borough, East Herts District, and Welwyn Hatfield Borough. You will be responsible for the delivery of care to an allocated caseload of service users experiencing, or recovering from, mental health difficulties. You will also be completing holistic assessments, developing innovative support plans, reviews and CPA in conjunction with other professionals while building networks, raising awareness in the area and developing valuable links for the team. You will be working alongside Consultant Psychiatrists, Psychologists, Nurses, Occupational Therapists and Support Workers to achieve the best outcomes for the individuals you work with. You will provide direct support and advice to the service users, their families and carers, within the recovery framework, embracing the individual right to choose and control. You MUST BE registered with Social Work England in order to apply for this role (Previous applicants need not apply). This role attracts a Golden Hello of £3,500, new employees only, with £1,500 on commencement and £2,000 on completion of one year's employment. Main duties of the job

You will be required to demonstrate your knowledge of the relevant legislation, theory and policy in relation to the service user group, such as The Care Act 2014, The Mental Capacity Act 2005 and The Mental Health Act 1983 amended in 2007. To be a champion of social care with a comprehensive understanding of legislation and statutory guidance. To take a lead in the promotion of adult safeguarding practice and undertake safeguarding enquiries. To undertake comprehensive assessments and reviews for people who may present with a high risk. To offer direct support to service users and their carers, and to colleagues, through information and advice. To ensure that appropriate health and social care services are commissioned with and on behalf of service users. To undertake regular risk assessments and ensure that vulnerable people are kept safe. KNOWLEDGE, TRAINING AND EXPERIENCE

An appropriate professional social work qualification (e.g. DipSW, Education to degree level). Registration with Social Work England. Training in principles and application of outcomes focussed assessments and personalization. Relevant post-qualifying training for social workers. Training and experience in Safeguarding. Approved Mental Health Professional/Best Interests Assessment Training. AREAS OF EXPERIENCE AND KNOWLEDGE

Minimum of 2 years post qualification experience. Knowledge of legislation, theory and policy in relation to relevant care group. Experience of undertaking assessments, focussed as personalisation, choice and control, and care planning. Understanding and experience of personal budgets and direct payments. Experience of multidisciplinary working and ability to work collaboratively. Experience of partnership working with service users and carers, demonstrating dignity and respect. Experience of work as Approved Mental Health Professional and/or Best Interests Assessor. COMMUNICATION SKILLS

Excellent ability to communicate with a wide range of stakeholders including service users and carers, other professional colleagues, members of other organisations and local communities, verbally and in writing. Experience of using electronic recording systems and ability to produce accurate, appropriate and timely reports. ANALYTICAL SKILLS

Ability to analyse service user and carer information. Experience of undertaking small scale audits and surveys. DIVERSITY

Demonstrable ability to work sensitively with diverse needs and environments. Commitment to anti-discriminatory practice. MENTAL & EMOTIONAL EFFORT

Flexibility and ability to respond to a variety of tasks, and to prioritise own workload. Ability to deal with distressing and sometimes aggressive behaviour. GENERAL

Experience of working in an integrated health and social care environment. Disclosure and Barring Service Check
