Children with Disabilities Social Worker
to work for our client based in
Ryde . Role Responsibilities
You will be working with children who have severe and complex needs including Autism Spectrum conditions. The team undertakes Child and Family assessments of children who are disabled and their families. This includes working with families to address safeguarding concerns and working with Children in Care, whether full time or short break care. Cases can be managed under child in need which can include short breaks out of the home and the provision of direct payments. The work tends to be long term in nature. A lot of our work can be multi-disciplinary and requires a coordinated response between services. This might include collaboration with Health, Children and Young People’s Continuing Health Care, SEN Case Workers, Specialist Schools, and other clinical and therapeutic practitioners. We also work closely with our colleagues in Adult Services to ensure young people are supported to transition successfully into adulthood.
