Responsibilities: Respond to and meet the identified needs of children/young people as outlined in individual care plans and as agreed with the manager. Work in partnership with young people, their families, social workers, and other relevant agencies to meet the needs of young people. Ensure that work with children/young people is in keeping with legislative, procedural, and good practice requirements. Ensure that the safety and welfare of young people is paramount. Participate in implementing, monitoring, and reviewing children/young people's care/health and education plans where appropriate and contribute in planning meetings and reviews as required. Understand and be familiar with all relevant legislation, guidance, policies, and procedures in relation to safeguarding young people. Promote the development of a comfortable physical environment, conducive to good social care practice and in line with standards and guidance. About Acrefield: Acrefield House offers individual personal living spaces for privacy, dignity, and comfort, with shared communal areas to promote interaction, inclusion, and enjoyment. Located in Birkenhead, Acrefield contains 5 bedrooms and supports young people from the ages of 16 to 25 with very complex needs, including autism, learning disabilities, and mental health issues.
