This information is removed before shortlisting. All Age Continuing Care (AACC) have opportunities for band 6 Senior Practitioners to join our Clinical Bank. The post holder will play a key role across the CHC service, working within the remit of the National Framework for Continuing Healthcare, to deliver an effective, efficient and equitable service to the people and populations of Sussex. Main duties of the job

Prior experience of CHC would be beneficial, but is not essential for this role, as you will work within an experienced and supportive team who will support you to develop the relevant knowledge required. The post holder should however have experience of working within health or care, and will need excellent verbal and written communication skills, as you will be required to work closely with patients, families, carers and other external partners. As a Senior Practitioner, your role will include delivery of a variety of functions across the CHC service, including: Assessment Reviews Care Planning Case Management You will also have the opportunity to work across other areas of CHC, including audit, appeals, retrospective assessments, complaints, assessment verification, and many more. You will be supported to develop the necessary skills and experience to work across the whole pathway, safely, confidently and effectively. Candidates would need to be able to work a minimum of 3 days a week on the bank and would need to be experienced in continuing healthcare. About us

The role will require travel on occasions, to carry out assessments and reviews. Access to transport is therefore necessary, be it personal or public. Experience

Experience of working in a health or care environment (including acute, community, mental health, local authority etc.) A good understanding of the health and social care environment and roles and responsibilities within it. Negotiation, conflict management and influencing skills. Involvement in operational processes and evidence of using a problem solving and forward thinking approach. Skills and Knowledge

Skills in patient assessment interpreting both physical and mental health needs and formulating care plans to meet identified needs. Demonstrates ability to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales. Ability to address complex or sensitive issues in an organised and appropriate manner both verbally and in writing at all levels; written documents, spreadsheets. Knowledge and experience of working with Safeguarding issues and Policy guidance. Knowledge of the Mental Capacity Act and Best Interests decision making. IT literate and working knowledge and computer proficiency of all Microsoft Office packages (e.g. word processing, spreadsheets, e-mail and internet use). Intermediate keyboard skills. Knowledge and understanding of current national and local policy/Framework Guidance relating to NHS Continuing Healthcare and NHS Funded Nursing Care. Understand and be able to identify and support the Freedom of Information and Subject Access Requests as required. Qualifications

Educated to degree level or relevant experience in Nursing With subsequent up to date registration with the relevant regulatory body the Nursing & Midwifery Council (NMC), Evidence of Continued Professional Development Disclosure and Barring Service Check
