Thepost holder will be required to provide a high-quality resource of professionaladvice, guidance and expertise to members of the Individual Care Team, ICB andwider health and social care economy Autonomouslymanage a case load of individuals with highly complex health needs providingsignificant clinical guidance to providers. Ensurethat all assessments are holistic in nature and utilise The National Frameworkfor NHS Continuing Health Care. Tohave an expert level of knowledge regards The National Framework and itsapplication in practice. To be able to impart this knowledge to a variety ofaudiences. Toprovide complex and sensitive information to patients, relatives/representatives and often work in potentially hostile environments; relayingdifficult decisions regarding care provision and managing patient and familyexpectations. Providecontinuity of care throughout the period of care and support. This willencompass close working with health and social care professionals and careproviders. Attendall meetings related the patient which may impact on the care provision andcosts to meet identified needs Liaisewith multiple professionals involved in the patients care to ensure that acomprehensive understanding of all identified needs is available. Ensuring thateach patient has a care plan that addresses all their identified needs and canbe used to assist assessors when completing CHC assessments/reviews. Presentcomprehensive written reports to ICB Eligibility Panel following any reviewscompleted once patients are in receipt of agreed care provision that identify achange in the level of care required. This includes being able to demonstrateimprovements that may result in a reduction of care provided or a change thepatients eligibility for NHS CHC. Tobe responsible for negotiating care package costs and the monitoringof packages of fully funded care which are in excess of £1000 perweek. To be responsible for ensuring these care packages are reviewed to anagreed time scale, identified by the Head of Service and that these arerecorded on the ICB database. Identifyand process any safeguarding and quality of care issues and takeappropriate action to ensure that peoples welfare is protected and thatthe quality and standard of services provided are at a level that is inline with ICB required standards. Close liaison with the ICB Quality Team isnecessary to ensure ICB funded individuals are in receipt of high quality, safecare. EnsureMental Capacity Act is considered and ensure relevant processes are followedeg. Best Interests, DOLs Developand maintain effective working relationships with clinicians, generalpractitioners and providers in order to ensure continuity of care and addresspatient needs. Providea high-quality resource of professional advice, guidance and expertise to the ICBand partner agencies in addressing the needs of individuals with complex healthneeds. Liaisingwith a variety of providers and arranging assessment of patients in order thatthe most appropriate placement/provider is identified. Provideinformation and support to the senior managers when considering the mostappropriate provider to address a patients identified needs and to actautonomously when required to agree care packages in excess of the usual ICBcost. Tomaintain a formal record of all cases and ensure that all relevant clinicalinformation is available and updated regularly, both electronically and inpatient files. Supportthe Head of CHC in working with partner agencies and other stakeholders indeveloping the local market for services to ensure that it reflects therequirements of individuals and is able to meet current and future needs Tocall and chair multi-disciplinary meetings, including case conferences anddischarge meetings to ensure continuity of care across agencies; thus improvingpatient and family experience. Deputisefor the Lead Nurse as required Providedata in order that the ICB can continuously monitor care delivery and identifyhow performance of providers can be improved. Working closely withcolleagues in the ICB Quality Team to assure the quality of care received byNHS funded individuals. Tohave budgetary monitoring responsibilities and to ensure that the Head ofService is informed of any identified budget pressures or financial risks thatmay impact on the overall service budget. The post holder will need todemonstrate both quality of care and value for money in all placements. Todevelop policies and procedures, as requested, which will support the overalldelivery of NHS Funded Care within the ICB. Tocontribute to and deliver training on CHC and FNC to a variety of audiencesparticularly in health and social care. Toprovide professional support and mentoring to the nurse assessors within theteam and provide additional support and training as required by the Lead Nurse.
