The post holder is responsible for the delivery of general practice nursing services care to the practice population. Supported by experienced practice nurses within the Practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as required. They will be requested to do one late night.

Undergo relevant training to support and advise women requesting information relating to family planning needs. Undergo relevant training to support and manage the health needs of women presenting for cervical cytology consultations. Under the direction of experienced practice nurses, implement and participate in vaccination and immunisation programmes for both adults and children. Advise, support and where appropriate, administer vaccinations for patients travelling abroad. Person Specification

RGN with current NMC registration. Evidence of Continued Professional Registration. Recognised qualifications in diabetes or chronic heart disease or respiratory medicine. A diploma in General Practice Nursing. Previous Experience in Primary Care. Qualifications/Certificates in Cytology, Travel vaccinations, Immunisations. Experience
Working within Primary Care setting. Nursing management of chronic disease conditions in general practice, i.e. Diabetes, Asthma/COPD, Heart Disease. Knowledge and Skills
Excellent communication skills, both written and verbal. Change-management skills and ability to support patients to change lifestyle. Ability to apply clinical theory to practice.
