As a Community Support Worker, you'll play a vital role, working closely with the multi-disciplinary team made up of clinicians such as Physiotherapists and Nurses to provide ongoing clinical activities such as nursing tasks, rehabilitation, recovery and support with daily routines and personal care. Successful candidates will have the support from their teams to be comfortable working independently in the community. Undertake a range of delegated clinical, therapy & nursing led activities to support the service users on the Integrated Network Team caseload. Provide safe, patient-centred and effective clinical activities in the community setting. Work within adult services to deliver a full range of healthcare activities. Work flexibly with other teams or services, both within the locality and Sirona wide when the need arises at short notice. Be able and willing to travel throughout the service delivery geographical area. Enhanced pay for weekends and bank holidays, mileage is reimbursed whilst you carry out your duties at work. We also offer generous annual leave allowance and an attractive staff benefits scheme. Qualifications

NVQ Level 3 in Health & Social Care or willingness to work towards. Skills, Knowledge & Experience

Able to work without direct supervision, as agreed within the team, to achieve the objectives of the post. Able to prioritise tasks and make correct decisions within a set of clear guidelines. Experience of direct patient care in a health or social care setting. Ability to manipulate fine tools e.g. for venepuncture. Experience of team working and an understanding of team dynamics including what factors make a team work well and what can go wrong and why. Able to use a variety of IT systems e.g. email, EMIS. Recent relevant experience within a community setting. Application Reason
