Non-clinical Assessors

who are committed to enhancing the quality of life, to be based at our

Southport Location . Our new Single Aged Care Assessment System simplifies the journey for elderly persons seeking care, ensuring their needs are met efficiently and effectively. Responsibilities: Conduct home support assessments using the Integrated Assessment Tool (IAT). Collaborate with clinical assessors to address clinical questions during assessments. Develop Support Plans that reflect the client's goals and needs. Record assessment information accurately and comprehensively. Role Requirements: Minimum 1 year case management experience in assessment and goal-orientated support planning in aged, community, health preferred. Previous knowledge or work experience in the aged care system preferred. Unrestricted Australian Drivers Licence. National Police Check. Access to a motor vehicle with full comprehensive insurance.
