To work within the Domiciliary Care Service undertaking initial client assessments and screening initial referrals received via Local Authority or directly from clients enquiring about services provided. The post holder will be required to assess and determine the nature and level of care required by individuals referred to the service. Qualifications
Level 4 in Health and Social Care or equivalent level of knowledge, skills, and experience Care Certificate or willingness to undertake this within the induction period Knowledge
Knowledge of planning and completing initial client assessments Knowledge of policies, procedures, and practices relating to domiciliary care Knowledge of a range of software programmes including Microsoft Word, Excel, and Outlook Knowledge of completing risk assessments appropriate to domiciliary care Knowledge of relevant legislation e.g. Mental Health Act (1983), Mental Capacity Act (2005), Safeguarding Adults, and Deprivation of Liberty (DOLS) Clear understanding of domiciliary care and client needs in the community Knowledge of databases used within domiciliary care Experience
Experience of completing initial client assessments either in the community or within a residential setting Experience of working in a health and/or social care setting Experience of undertaking risk assessments Experience of keeping accurate records and inputting data into relevant database systems Experience of undertaking a similar role within domiciliary care services Experience of working with people with a range of health conditions such as dementia Experience of working with people with end-of-life needs Experience of liaising and working with multidisciplinary teams or care teams Disclosure and Barring Service Check
