You will be part of a team providing help and confidence to those you are supporting to access the right help across our complex systems. This will involve working in the community with Children and Young People and their Parents/Carers and collaborating with a range of specialists across our region. Working within the BaNES, Swindon and Wiltshire's (BSW) Integrated Care Board Learning Disabilities and Autism department, you will be part of a team of Keyworkers covering the BSW area led and supported by the Keyworker Manager, Team Lead, and Keyworker Specialist. Main duties of the job
The Keyworker Function supports children and young people living within BaNES, Swindon and Wiltshire with a learning disability, autism or both, with the most complex needs to access the right support. Key responsibilities include: Managing a caseload of children and young people who have been identified as requiring a keyworker. Building and maintaining relationships with Children and Young People and their Parents/Carers in clinical, community, and home settings. Working with partner organisations, particularly within Education, Social Care, and Healthcare to navigate local services and pathways. This post holder will: Manage a caseload of children and young people (CYP) who have been identified as requiring a keyworker. Facilitate the provision of emotional and behavioural support and informed advice to CYP and their families/carers. Create and encourage a culture which prioritises keeping CYP within the home and their local community. Work in partnership with organisations within the integrated care system (ICS). Facilitate transition to a support worker within the team to enable a step-down process. Person specification
Knowledge, Training and Experience
A proven understanding of the needs and characteristics of children and young people with a diagnosis of Autism and/or complex communication difficulties. Experience of working with children and/or young people with learning disabilities, autism, or both. An understanding of equal opportunity issues. Skills and Abilities
Empathy and understanding. Respect for the opinions and beliefs of children, young people, and their families. Commitment to ensuring the rights of young people with learning disabilities and/or autism are protected. Experience
Experience of working with children/young people with Autism and/or learning disabilities. Experience of liaising with multi-disciplinary professionals. Education
Relevant Professional qualification, e.g., social worker, nurse, occupational therapist, etc. Evidence of post-graduate training specific to Autism and Learning Disabilities.
