The role will be predominantly remote but with the expectation of attending the site for commencement of the post.
The successful post holder must have experience in General Practice, have completed the PCPEP course and be an independent prescriber. Those who do not meet these criteria need not express interest or apply due to the short-term nature of the role.
The role is paid at the
equivalent
of NHS agenda for change band 7-8a, depending on experience.
Main duties of the job Purpose of the role
To support Aire Valley Surgery with all aspects of medicines optimisation.
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the practice. This will include structured medication reviews with direct patient contact and may include contributing to:
Management of long-term conditions
Management of medicines on transfer of care
Reviewing systems for safer prescribing
Contributing to repeat prescription authorisations and reauthorisation
Addressing both the public health and social care needs of patients
Contributing to achievement of QOF and locally commissioned quality improvement schemes
Undertaking clinical audit
The post-holder will provide support to pharmacy technicians and less experienced pharmacists employed in the practice.
The post holder will be supported by the Lead Practice Pharmacist. They will also engage with pharmacy colleagues in Yeadon PCN and across Leeds for peer support.

Demonstrable experience as an established foundation-level pharmacist

Knowledge of IT systems, including ability to use word processing skills, emails and the internet
Able to communicate complex and sensitive information in an understandable form to a variety of audiences
Qualifications
Professional registration with the General Pharmaceutical Council
Completion of an undergraduate degree in pharmacy
Holds or working towards an independent prescribing qualification
Membership of the Royal Pharmaceutical Society
Experience
Demonstrable experience as an established foundation-level pharmacist
Experience and awareness of common long-term conditions
Experience of partnership/collaborative working across various organisations
